Research Description
My laboratory is focused on gastrointestinal mucosal inflammation and carcinogenesis. We study the host immune response to the gastric pathogen Helicobacter pylori that causes peptic ulcer disease and gastric cancer. This includes the innate immune response in macrophages and the identification of ways that this response is ineffective. We have also elucidated mechanisms whereby epithelial responses are inappropriate, leading to risk for cancer development. We study novel mechanisms for inflammation and cellular/DNA damage, which leads to increased cancer risk. We focus on aberrant signaling in macrophages and epithelial cells that leads to persistent inflammation and cancer risk. Polyamines are a major focus in our Lab.

We have reported that H. pylori induces the enzyme spermine oxidase (SMOX), which utilizes the polyamine spermine as substrate. Generation of H2O2 by SMOX results in apoptosis of macrophages due to mitochondrial membrane depolarization. SMOX causes oxidative stress in gastric epithelial cells that leads to both apoptosis and DNA damage. We recently reported that Smox-/- mice have reduced gastric inflammation and carcinogenic signaling with H. pylori infection (Oncogene, 2020). Induction of ornithine decarboxylase (ODC) that generates polyamines also leads to apoptosis of macrophages, contributing to loss of host innate immunity to H. pylori. We reported (PNAS 2017) that myeloid-specific knockout of Odc leads to increased immune activation and inflammation in the mouse H. pylori infection model. We recently reported (Cell Reports 2020) that a novel pathway for protein translation inmacrophages, hypusination, is very important in the innate immune response to H. pylori. We also recently reported (J immune 2022) that gastric epithelial-specific knockout of Odc leads to protection from gastric inflammation and immune cell activation. We also reported (JCI Insight 2022) that the enzyme cystathionine gamma-lyase (CTH) is upregulated in H. pylori infection and that its deletion leads to diminished gastric inflammation and alters immunometabolism in macrophages, making it a potential therapeutic target.

Another discovery in our lab was that epidermal growth factor receptor (EGFR) phosphorylation is an important part of the host response to activating stimuli, such as H. pylori or the colitis-inducing bacterium, Citrobacter rodentium, in macrophages. Mice with myeloid-specific deletion of EGFR exhibited decreased H. pylori-induced gastritis and C. rodentium-induced colitis (J Clin Invest, 2016), and less tumors in the AOM-DSS model of colitis-associated carcinogenesis (Oncogene, 2017). Further, we also reported that an EGFR inhibitor given in the food markedly attenuated gastric carcinoma development in the mouse INS-GAS and gerbil models of H. pylori-induced cancer (Gut, 2018).

We are extensively utilizing clinical material from cohorts of human subjects from Colombia and Central America, where H. pylori infection prevalence is very high, but gastric cancer risk is much higher in the mountains when compared to the coastal regions. Our studies implicated the polyamine-driven oxidative stress and associated DNA damage. Work showing that an inhibitor of polyamine synthesis reduced gastric inflammation and cancer in rodents (Oncogene, 2015) led to an R01-funded clinical trial that we are conducting with this agent (difluoromethylornithine) in human subjects with precancerous gastric lesions.

We are also pursuing studies on immune dysregulation in inflammatory bowel disease. This includes work on arginine availability/transport, polyamines, nitric oxide. and the microbiome. We reported that levels of the amino acid L-arginine are decreased in the tissues of humans with ulcerative colitis (IBD Journal, 2016). Consistent with this, we have shown that treatment with L-arginine improves colitis in mouse models. Further, mice lacking an inducible L-arginine transporter have increased disease activity in the DSS injury model but reduced colitis in the C. rodentium infection model. We have recently investigated alterations in the microbiome as a potential contributor to these differences (Front Cell Inf Microbiol, 2019). We also showed that Slc7a2-/- mice have increased tumorigenesis in the AOM-DSS model (Oncogene, 2019). We reported (Gastroenterology 2022) that the polyamine spermidine is protective in the DSS mouse model of colitis and in the AOM-DSS model of colitis-associated cancer and in an Apc deletion model of sporadic colorectal cancer. We also reported (Gastroenterology, 2023) that mice with intestinal epithelial-specific deletion of deoxyhypusine synthase (DHPS) an enzyme that causes hypusination, a specialized protein translation pathway, exhibit spontaneous colitis and severe chemically induced colitis, and increased tumor formation with a carcinogen; in parallel human tissues from inflammatory bowel disease patients show low DHPS and hypusination.

We also recently described (Gastroenterology, 2021) a mechanism of inflammation-associated carcinogenesis related to dicarbonyl electrophiles. These molecules form adducts to proteins and DNA, which we demonstrated in H. pylori infection of the stomach and in colitis and associated carcinoma; a scavenger of electrophiles reduced gastric or colon carcinogenesis. We have gone on to show that a clinically-available electrophile scavenger (2-hydroxybenzylamine) prevents gastric cancer (Biomed Pharmacother, 2023) and colorectal cancer (Oncogene, 2023) in mouse models. We recently were awarded a patent for this work.

2) Oxidative stress and electrophiles in the development of gastric and colorectal cancer (NIH R41 grant).
3) Role of polyamines, hypusination and SMOX in epithelial dysfunction in H. pylori infection and carcinogenesis (PI, project on P01).
4) Inhibition of polyamine synthesis with difluoromethylornithine in human subjects in Colombia and Honduras with precancerous gastric lesions (PI, R01 grant; funding completed, analysis ongoing from clinical trial).
5) Role of electrophiles in colorectal cancer and colitis (PI, Department of Defense Impact Award).

There are opportunities to study mucosal immunology, inflammation, and carcinogenesis. Expertise in molecular biology, immunology, microbiology, and/or biochemistry is highly desirable. Candidates with an interest in host-pathogen interactions in the gut can develop those interests in the Wilson Lab. There are opportunities to conduct research using rodent and in vitro models, and translational studies, especially working with organoids from stomach and colon, as well as multi-comics.
